This offer is not contingent on any past, present or future purchase, including refills. This REBYOTA Co-pay Program offer is good for use only upon the administration of REBYOTA (“Product”) for an FDA approved indication in the US. The value of this program is exclusively for the benefit of patients and is intended to be credited towards patient out-of-pocket obligations for REBYOTA including applicable co-payments, coinsurance, and deductibles. Ferring will provide coverage maximum cap of up to $5,000 per dose with a maximum benefit of $10,000 per patient per calendar year. The value of this offer may not exceed the amount of the patient’s out-of-pocket costs for the Product. Documentation including Explanation of Benefits (EOB) will be required. The REBYOTA Co-pay Program is not health insurance. Offer is invalid for all transactions and claims submitted more than 180 days following the date of service. The REBYOTA Co-pay Program is not valid for Product reimbursed in whole or in part by Medicare, Medicaid, or a Medicare Part D plan, TRICARE, US Department of Veteran Affairs, US Department of Defense, the Commonwealth of Puerto Rico, government health insurance plan, or any other federal or state-funded healthcare benefit program. The REBYOTA Co-pay Program is void where prohibited by law, taxed, or restricted, as in the States of California and Massachusetts. The REBYOTA Co-pay Program is not transferable and is limited to one program benefit per patient. No substitutions are permitted. The REBYOTA Co-pay Program benefit cannot be combined with any other co-pay program, free trial, discount, prescription savings card, or other offer. Patients, pharmacists, and prescribers cannot seek reimbursement from health insurance or any third party for any part of the benefit received by the patient through this offer. Ferring Pharmaceuticals reserves the right to rescind, revoke, or amend this offer without notice. Offer expires 12/31/24.

REBYOTA Patient Assistance Program^a

You may be eligible to receive REBYOTA at no cost if you qualify for the Ferring REBYOTA Patient Assistance Program (PAP). Simply ask your doctor’s office to enroll on your behalf through REBYOTA Connect.

The REBYOTA Patient Assistance Program is only available for qualified patients enrolled in REBYOTA Connect.

Participation in the PAP is free. Ferring does not collect any fees from people seeking assistance through PAP.

Assistance is provided in the form of free product shipped to site of administration or reimbursement for the invoiced cost of the product if processed after product has been administered. If assistance is in the form of free product, REBYOTA must be secured at the site of administration until the qualified PAP patient’s treatment date, and the medicine must be provided only to the eligible patient at no charge of any kind for the product. If product administration occurs and patient was qualified for PAP, reimbursement for the invoiced cost of the product can occur for Healthcare Providers or Specialty Pharmacies after they have provided supporting documentation for the product cost only in addition to a completed Reimbursement Request form. The REBYOTA PAP only covers the cost of the product and not administration or other services.

Any medications supplied by Ferring as a result of enrollment in the PAP are for the use of the named patient only and will not be sold, traded, bartered, transferred, returned for credit, or submitted to any third party (such as Medicare, Medicaid, or other benefit provider) for reimbursement.

If REBYOTA is no longer medically necessary for a qualified patient’s treatment or if the patient’s insurance or financial status changes, this must be reported to REBYOTA Connect.

If patient authorization is signed, electronic verification of a patient’s financial status is available to help determine eligibility,

PAP beneficiaries with government insurance are eligible to obtain medication through the end of the calendar year. PAP beneficiaries with commercial insurance are eligible to obtain medication for up to 12 months.

Healthcare Providers may not submit claims or payment from this medication through an insurance plan of any kind. Patients may not seek true out-of-pocket (TrOOP) or other insurance credit for medication provided through the PAP.

If applicable, PAP recipients will have their health plans receive written notification.

Ferring may change or cancel the PAP at any time and Ferring reserves the right to terminate a patient’s enrollment in the PAP at any time.

As a newlywed, having C. diff was devastating. I dreamed of being this beautiful princess bride and everything was going to go great. The first year was supposed to be this big honeymoon period, and that was blown out of the water.

Initial Symptoms 
I was constantly running to the bathroom 3 to 4 times an hour. I was experiencing a lot of pain in my gut, which was new. The combination of nausea and pain was scary to me.

C. diff Diagnosis 
I had just recently been discharged from the hospital for another underlying condition. My C. diff doctor suspected that there might be more going on, so I was doing a lot of follow-ups.

C. diff Recurrence
I knew I had a recurrence of C. diff following bloodwork and other testing.

Getting REBYOTA
My C. diff doctor was the first person who brought up REBYOTA as an option. I went to the REBYOTA website to learn more about it and how it works.

My doctor’s office reached out to my insurance and REBYOTA was 100% covered by the government insurance I had at the time.

REBYOTA is available by prescription only. You should not receive REBYOTA if you have a history of a severe allergic reaction (e.g., anaphylaxis) to REBYOTA or any of its components.

After Administration With REBYOTA
Once I finished receiving REBYOTA, my nurse left the room. I laid on my side for about 15 minutes, and then I was able to drive myself home afterwards.

I was so surprised because over the next few days I noticed a significant decrease in the number of times that I was rushing to the bathroom. The intensity seemed to subside significantly. As each day went on, I felt incrementally better, which hadn’t happened in so long.

Treatment times vary. Administered after completing a course of antibiotics for recurrent C. diff infection.

Life Since REBYOTA

It has been over six months since REBYOTA, and I have not had any recurrence of C. diff. When I leave the house now, I do not have any of the worries I used to.

Recently, I was so excited because my husband and I went to the Phoenix State Fair. We were there from opening at 10 AM to 10 at night. Six months ago, there was no way on earth I would have even conceived of going somewhere like that.

Individual results may vary. Patients should not take any oral antibiotic therapy for up to 8 weeks after administration of REBYOTA unless directed by their doctor.

INDICATION

REBYOTA (fecal microbiota, live – jslm) is indicated for the prevention of recurrence of Clostridioides difficile (C. diff) infection in individuals 18 years of age and older, following antibiotic treatment for recurrent C. diff infection.
Limitation of Use
REBYOTA is not indicated for the treatment of C. diff infection.

Important Safety Information

You should not receive REBYOTA if you have a history of a severe allergic reaction (e.g., anaphylaxis) to REBYOTA or any of its components.
You should report to your doctor any infection you think you may have acquired after administration.
REBYOTA may contain food allergens.
Most common side effects may include stomach pain (8.9%), diarrhea (7.2%), bloating (3.9%), gas (3.3%), and nausea (3.3%).
REBYOTA has not been studied in patients below 18 years of age.
Clinical studies did not determine if adults 65 years of age and older responded differently than younger adults.
